ラストメッセージ(4)ガンと闘うな、痛みと仲良くなれ

〔末期ガンをサーフする3〕

ステージⅣの食道ガンが見つかって10か月がたとうとしている。
先日は市役所に出すための介護保険の申請書を書いたが、病名を「末期ガン」と記入した。
「末期」である。
このあとはない、ということだ。

担当医とも当初から「根治をめざさない」という方針で同意し、そういう方向で治療をすすめてきた。
治療といっても、ガン根治が目的ではなく、日常生活ができるかぎり快適にすごせるようにするための治療である。

私のニーズは「生きのびる」ことではなく、「ガンを根治する」ことでもなく、「できるだけ生活のクオリティを落とすことなく毎日を快適にすごし、日々の時間をじっくりと味わいながら、やりたいことを心おきなくやりつくすこと」である。
「死ぬときまでできるだけ生き生きと生きること」である。
そのことは複数いる担当医も合意してくれている。

合意に至る前の、セカンドオピニオンをもらった医師のなかには、従来のような暴力的で強圧的な「あんた、のんきなこといってると死ぬよ。なんとしてでも生きぬく、その意欲を示してもらわないと、医者もそれに応えられないんだよ」と迫った者もいる。
その医師の熱意はありがたかったし、ある意味では職業倫理の深いすばらしい医師なのかもしれなかったが、いまの私には同意できない。

私はガンと闘わないことを選択した。
ガンと闘うことを選択するというのは、まずは抗ガン剤、そして放射線治療、摘出手術、さらに重ねて抗ガン剤という標準治療といわれる手順=ベルトコンベアに乗ることだ。
私はそれを選ばなかった。
近代的な専門病院や専門医は、そんな患者の意志を最大限尊重してくれることもわかった。

根治を目的としなくなったとき、治療が目的の消化器外科や化学療法科や放射線科といった病院の治療部門は、やることがなくなる。
ガンがどの程度進行しているのか、お願いすれば検査くらいはしてくれるかもしれないが、そもそも治療をしないので一見かなり冷たい対応となる。
しかし、こちらにはまだ気がかりがあって、快適に日常生活をいとなむには、たとえば痛みの問題があったり、食事や排泄に支障があればそれに対処したかったりする。

私の場合、食道にガンがあって、それがしだいに肥大して食道をふさいでいった。
食べたり飲んだりしたものが食道を通りぬけにくくなってきたのだ。
そのため、抗ガン剤による治療はおこなわず、また摘出手術もしなかったが、放射線照射治療はおこなった。
医師が食道閉塞には効果的だろうという判断をしたからで、私もそれに同意した。

食道のガン部位への放射線照射治療は全部で30回おこなった。
その結果、ガン部位はかなり縮小して、飲食にはほとんど支障がないほどまでに回復した。
これはありがたかった。
なにもしなければ食道はやがて完全にふさがり、水も通らなくなっただろう。
胃瘻をおこなうか、ステントを入れて強制的に通り道を確保するしかない。
あとは栄養失調で死にいたる。

食道への放射線治療はうまくいったが、今度は腰と下腹部への痛みが出てきた。
腹部大動脈の脇にあるリンパ節に転移があって、そちらが増大化・肥大化しているらしいが、痛みはそれが原因かどうかよくわからないという。
たしかに、腰とか下腹部はリンパ節から離れているような気がする。
医者も首をひねっている。
が、効果があるかもしれないということで、腹部への放射線照射治療を10回だけおこなうことになった。

現在、その治療が終わって10日ほどたつのだが、目立って効果が出たようには感じない。
つまり、痛みは相変わらずあるし、むしろ強まっているような気もする。
(ゆっくりとでもいいので効果が現れてくれるとうれしいのだが)

やれることは全部やったので(放射線治療はおなじ箇所には二度とできない)、あとは対症療法で毎日が快適にすごせるように工夫するのみ。

もっかのところ、最大の問題は痛みだ。
痛みがあると集中力もなくなるし、そもそもやる気がいちじるしくそがれる。
なにもやる気になれない。
ご飯もおいしくないし、ただじっとしているだけでもしんどい。

幸い、近年の医療は緩和ケアが進んでいて、さまざまなペイン・コントロールの方法や薬が出ている。
私は医療用麻薬のオキシコンチンを処方してもらって、最初は5ミリグラムを日に2回飲んでいたが、昨日から10ミリに増量した。

麻薬というとなんとなく聞こえが悪いのだが、痛みは緩和されるし、思ったほど副作用は強くない。
それほど眠くもならない。
集中力はそがれないし、感覚も鈍くならない。
鈍くなるのは痛みだけで、その他の感覚はいつもどおりというか、むしろ鋭敏になるくらいに思える。

完全な健康体とおなじ快適さとまではいえないが、ある程度快適な状態で私はやりたいことができる。

さて、ペイン・コントロールがある程度できて、ほかにはとくにやるべきことがないという状況のいま、私はなにをやりたいのか。
なにができるのか、そしてそれは本当にやりたいことなのかどうか、残り時間がどのくらいなのかわからないが、あまり多くはないかもしれない時間を使ってやるだけの価値が自分にとってあることなのかどうか。
私の生命が欲している本当のニーズはなんなのか。

これから私はそのことについて、自分の存在そのもの——とくに身体——とじっくり対話を重ねてみようと思っている。
いったい私自身はどんなことを望んでいるというのか、身体はどんなふうに答えてくれるのだろうか。

それを実行した結果、私にはどんなことが起こるのだろうか。
ガンは変化するのだろうか。


It has been 10 months since stage IV esophageal cancer was found.
Recently I wrote an application for nursing care insurance to submit to the city office. The name of the disease was as "terminal cancer".
“Terminal stage".
There is no more.

From the beginning, the doctor in charge agreed to the policy of "have no cure" and recommended treatment in that direction.
The goal of treatment is not to cure cancer, but to make the quality of  life as comfortable as possible.

My need is "To spend every day comfortably without deteriorating the quality of life as much as possible, and to do what you want to do without minding, while enjoying the time of each day.” Is not "survive" or "eradicate cancer".
“The act of living as vividly as possible until one's death".
Several doctors agreed.

One of the doctors who received a second opinion before reaching an agreement urged them to "You're going to die if you take it easy. You can survive by any means, and if you don't show your willingness, the doctor won't be able to respond." as violent and coercive as before.
The doctor's enthusiasm was appreciated, and in a way, he might be a great doctor with a deep professional ethics, but I cannot agree with him now.

I chose NOT to fight against cancer.
The choice to fight cancer is to go on a conveyor belt, a standard treatment regimen known as anticancer drugs, followed by radiation therapy, surgery to remove the cancer, and then finally anticancer drugs.
I didn't choose that.
It was also found that modern specialized hospitals and medical specialists respect the will of such patients as much as possible.

When curative intent ceases to exist, hospital treatment departments such as gastroenterology, chemotherapy, and radiology will have nothing to do.
They may be able to test how advanced the cancer is, if you ask them, but since they don't treat it in the first place, it's pretty cold.
However, I am still concerned, and in order to enjoy my daily life comfortably, I want to deal with problems such as pain, or problems with eating and excrement.


In my case, I had cancer of the esophagus which gradually enlarged and blocked it.
It became harder for food and drink to pass through the esophagus.
Therefore, the patient was not treated with anticancer drugs and did not undergo surgery, but was treated with irradiation.
The doctor decided it would be effective for esophageal obstruction, and I agreed.

A total of 30 radiation treatments were performed at the cancer site in the esophagus.
As a result, the area of the cancer has shrunk to the point where it hardly interferes with eating and drinking.
If nothing had been done, the esophagus would soon have become completely blocked and no longer able to pass water.
Gastrostomy or stenting to force access is the only option.
The rest die from malnutrition.

Radiation therapy to the esophagus was successful, but she now has pain in her lower back and lower abdomen.
The lymph nodes on the side of the abdominal aorta appear to be enlarged and enlarged, but it is not clear whether this is the cause of the pain.
Indeed, the lower back and lower abdomen seem to be separated from the lymph nodes.
The doctor is also twisting his neck.
But the potential benefit led to 10 doses of radiation to the abdomen.

Now, about 10 days have passed since the treatment was completed, but I don't feel it has been noticeably effective.
In other words, the pain is still there, and it seems to be getting stronger.
(I would be happy if the effect appeared slowly.)

I did everything I could (Radiation therapy can never be done in the same place again.), so all I have to do now is to use symptomatic treatment to make my life comfortable.

The biggest problem is pain.
When I'm in pain, I lose my concentration and motivation.
I don't feel like doing anything.
The food is not delicious, and just sitting still is tiring.

Fortunately, palliative care has advanced in recent years, and a variety of pain control methods and drugs are available.
I was prescribed oxycontin, a medical drug, and at first I took 5 milligrams twice a day, but from yesterday I increased it to 10 milligrams.

Drugs don't sound good, but they relieve pain and have fewer side effects than you might think.
I'm not that sleepy.
I can't lose my concentration, nor can I lose my sense.
It is only the pain that dulls it, and the rest of the sensation seems to be the same or more acute.

If it's quite as comfortable as a perfectly healthy body, what do I want to do in some comfort.

Now that I have some pain control and nothing else to do, what do I want to do?
I don't know what I can do, and I don't know if it's really what I want to do, or how much time I have left, but I don't know if it's worth it for me to spend time that may not be too much.
What is the real need of my life?

From now on, I'm going to talk about it with my very being, especially my body.
How will my body respond to what I want?

What will happen to me as a result of that?
How does cancer change?
